Social Determinants of Health and PRCs’ Approach
CDC Prevention Research Centers (PRCs) across the country are working to address social determinants of health (SDOH) in their communities. SDOH are conditions in the places where people live, learn, work, and play that affect a wide range of health outcomes, including disparities in chronic disease. A major goal for each PRC is to focus on how their communities experience health disparities and to develop evidence-based interventions to address those disparities. Read their latest program brief to learn how PRCs are addressing chronic disease-related SDOHs, such as the built environment, community-clinical linkages, food and nutrition security, social connectedness, and tobacco-free policies.
